Create Person User Defined Field
Set up a user defined fields as appropriate in Admin Setup User Defined Fields.
Populate the Person User Defined Field
In the Person profile, populate those fields as appropriate.
Create Ad Hoc report
Now create an Ad Hoc report to capture the data for export to CSV in Reports > Ad Hoc. The Data Area and View should be set to Person. Then Sort by Person as outlined in the screenshots below.
Specify the desired Report Columns, including the user defined fields as you have labeled them.
Paste Ad Hoc into Excel
Once you run the report, click on "Download as CSV" and then use that data as desired in your reporting criteria and government forms.
